A number of school districts have requested a form to assist them in evaluating Career and Technology programs.  This sample evalution form is utilized by Birdville Independent School District in Haltom City, Texas.  The form is provided only as an example of the factors Birdville considers when evaluating its programs.  School districts wishing to examine similar factors in their programs are welcome to adapt this evaluation form to meet their needs.
