Requirements
Incentive Plan – Part I Plan Chart
v The incentive plan (Attachment 2 – Excel spreadsheet)
must:
v Match the program narrative
v Part I Est. Funds Dist. Chart, must relate back to Part
I Plan Chart Tab
v Part II Est. Funds Dist. Chart, must relate back to
BS6100 Payroll Breakout Costs Tab
v Part I must be at least 75% of the award amount
v Teacher attendance at school is not an appropriate
quantifiable measure for criteria #1 or #2
v Campus rating cannot be used as the only item in
criteria #1
v If the incentive plan is changed all these items must
be updated if affected by the change
Contingency Plans
v There must be a contingency plan for both Part I and
Part II:
v Part I:
v Any undistributed funds must go back to teachers who
meet criteria #1 and #2.
v The applicant must state how the funds will be
distributed.
v These funds cannot be used for professional development
or other activities.
v Part II:
v The applicant must state how the funds will be
distributed.
v These funds can go back to staff that met the criteria
in Part II, be used for professional development, other activities or be added
to Part I incentives.
Incentives Less Than $3,000
Justification
for less then $3,000 incentive per teacher
If
the incentives in Part I are less than $3,000 the applicant must explain
why.
This
may just be because based on the number of eligible teachers and the award
amount there are not enough funds to pay a $3,000 incentives to all of the
eligible teachers in Part I.
Publish Incentive Plan
How
was the incentive plan published
A
school district must state how it published its incentive plan for public
viewing in the narrative, such as posting the plan on the school or district
website.
Supplement Not Supplant
Complete
supplement-not-supplant section.
The
application must state that these funds will be supplemental and not supplant
any current funding.
PS3300, Part 2: Performance Targets
Performance measures
For each performance measure the applicant
must:
v Complete Reporting Instrument/Tool and
v Complete Next Grant Year Target/Goal % or # column.
District and Board Approval
District level decision making committee
approval:
v Applicants must also show evidence of approval of the
incentive program from the district-level decision making committee.
Board presentation or schedule to present
to the board:
v Applications must also show evidence of the
presentation of the incentive program at a regularly scheduled board of
directors meeting or provide the date of the meeting when the program will be
presented. The board minutes of a prior board meeting or documentation
must indicate that there are plans at a future school board meeting to present
the incentive program.
Evidence of Teacher Participation
Teacher letters (min. 3):
v No fewer than three individualized letters from
teachers (not aides, parents, administrators, etc.) , outlining their
involvement in the process and their support for the program.
Campus vote or agendas and minutes:
v Minutes from each Campus Committee meeting, including
attendance records listing subject area and grade level of each teacher present
and meeting minutes from all planning meetings, OR a record of a campus
vote among all teachers approving, by at least a simple majority, the proposed
incentive plan.
List of committee members:
v The list must include: name, subject taught, grade
level taught, and # years w/campus.
