These twelve data elements are: FIRST-NAME, MIDDLE-NAME, LAST-NAME, GENERATION-CODE, SEX-CODE, HISPANIC-LATINO-CODE, AMERICAN-INDIAN-ALASKA-NATIVE-CODE, ASIAN-CODE, BLACK-AFRICAN AMERICAN-CODE, NATIVE-HAWAIIAN-PACIFIC-ISLANDER-CODE, WHITE-CODE, and DATE-OF-BIRTH.
