Dispute Resolution
A comparsion of complaint resolution systems has been developed to assist parents and schools better understand dispute resolution options. If you would like to discuss dispute resolution options with staff in the Complaints Management Unit, please call and leave a message with the toll-free Parent Information Line at 1-800-252-9668. Forms for the Child-Centered Special Education Process
The Legal Framework for the Child-Centered Special Education Process is a template in electronic format that summarizes state and federal requirements for special education by topic. It is an effective tool in managing and maintaining compliant records for student identification, eligibility, placement, participation, progress, achievement, accountability, and graduation.

Uniform Transfer Agreement Form for Assistive Technology Devices
The Uniform Transfer Agreement form has been developed pursuant to Texas Education Code §30.0015. Transfer of Assistive Technology Devices. and Texas Administrative Code §89.1056. Transfer of Assistive Technology Devices.

Written Summary of Restraint Use sample form
The Written Summary of Restraint Use sample form is aligned with the PEIMS 2003-2004 Record 435 (Special Education Child Restraint – Student) reporting requirement. Districts may use this form to inform parents about the use of restraint and also to assist in the collection of data to be reported through PEIMS. This form contains the documentation requirements specified in 19 TAC §89.1053(e)(3),(4),(5),(A-I). Districts will be required to submit 435 data with the June 2004 PEIMS submission.
