Objective | All youth with disabilities, beginning at age 14 and younger when appropriate, receive individualized, coordinated transition services, designed within an outcome oriented process which promotes movement from school to post-school activities.

Result 1
Transition planning for students with disabilities includes participation or other State Agency or linkages when appropriate.

Result 2
Transition services for students with disabilities are based on individual student’s needs and preferences and the IEP/ITP represents a coordinated set of activities within an outcome-oriented process.
