TO THE ADMINISTRATOR ADDRESSED:

SUBJECT: Report of Students who are Deafblind

Enclosed is information concerning the annual report of students who are deafblind. This census generates federal funds for deafblind services and is used for regional, statewide and national program planning. The Texas Education Agency contracts with the deafblind specialists at the education service centers to coordinate the census process.

Reporting instructions and computer-generated forms (green) for students who were previously reported are enclosed. Blank forms (white) are enclosed for students who are new to the program or who were not included on the 1999 Census. The pink forms are to be used to indicate that there are no students to report.

Please note that the GREEN forms contain information from last year that must be updated. The program must submit all GREEN form(s) and/or WHITE form(s), or the PINK form indicating no students with deafblindness to report. The data on the forms must be accurate for all eligible students as of December 1, 1999.

The deadline for submitting these updated forms to the education service centers is February 4, 2000.
