To the Administrator Addressed:

School districts and campuses across the state are accountable for meeting the state's standards of performance for students and for providing educational programs in accordance with federal and state statutes and regulations. The state's accountability system includes a variety of on-site accountability evaluations designed to provide feedback for improvement. One such evaluation is a survey of parents.

Enclosed are several documents: (1) a set of Parent Special Education Surveys, (2) a list of students whose parents will need to receive the Parent Surveys from your office, and (3) envelopes metered and pre-addressed to the Agency. A Spanish-language version has also been included. If additional copies of the Spanish-language survey are needed, please contact the Agency at the number below.

The Parent Surveys must be disseminated to all parents of the students listed on the enclosed "purposeful sample" as soon as possible. Please note that each survey should be attached to the Agency-addressed envelope. If your student list includes more than one student from the same family, please ensure that the appropriate number of survey instruments is distributed to that family.
