SUBJECT: Title II, Part B-Eisenhower Professional Development Program
Annual Evaluation Report for 1999-2000
Section 2401(b) of Public Law 103-382 requires the evaluation of the Title II, Part Bū Dwight D. Eisenhower Professional Development Program funded through the Division of Student Support Programs Standard Application System. The attached 1999-2000 Title II, Part B annual program evaluation report (SSP-019R00) must be completed and returned no later than August 15, 2000. All report data pertain only to those specific services or programs provided through the use of Eisenhower formula entitlement funds and local education agency (LEA) cost share funds during the year beginning July 1, 1999, and ending June 30, 2000.
The recipient of Eisenhower formula entitlement funds (either the LEA which conducts its own project or the fiscal agent which administers a shared services arrangement) is responsible for submitting the completed annual evaluation report to the Texas Education Agency by August 15, 2000. Failure to submit this report by the August 15, 2000, deadline will interrupt the flow of Eisenhower cash payments to the LEA.
Please read and follow the specific instructions for each part and do not leave any questions blank on the report. Instructions and definitions for use in completing the report are included in this packet. All parts of the report must include private nonprofit school participation data. Report forms may be returned by mail or fax.
Shared Services Arrangements
| The fiscal agent of a Shared Services Arrangement (SSA) will complete one composite report for Parts A and B for all member LEAs. However, Part C must be completed by each member of the SSA or by the fiscal agent of the SSA in consultation with the LEA, as appropriate. Member LEAs must submit Part C-Performance Indicators to the fiscal agent by August 1, 2000. The fiscal agent is accountable for ensuring accuracy of Part C for each member district prior to submitting the SSA report to TEA by the August 15, 2000, deadline. |
If the LEA did not receive any Eisenhower formula entitlement funds during the year beginning July 1, 1999, and ending June 30, 2000, please complete the cover page of the evaluation report according to these directions:
1. enter the LEA name;
2. enter the ESC Region number;
3. enter the county-district number;
4. write "No Program" beneath the county-district number;
5. enter the name of the contact person in the space at the bottom of the page; and
6. return the page by mail or fax to the Division of Student Support Programs.
