PEIMS Data Standards 425 DISCIPLINARY ACTION DATA

On September 5, 2011, Austin Student (Student ID 888000099) and Lea Pupil (Student ID 456000078) assaulted Johnny Achiever. Austin Student and Lea Pupil assaulted Johnny Achiever which resulted in bodily injury to Johnny Achiever. Austin student is in special education. On September 06, 2011, two conference meetings were held (one for each student and their parents) to provide the “due process” requirements for a behavior that requires a mandatory DAEP placement. As a result of each conference meeting, Lea Pupil was assigned to the district Off-Campus DAEP located at campus number 003 for 30 school days. Austin Student wasn’t assigned to the district Off-Campus DAEP because the behavior was a manifestation of his disability. The information for Austin and Lea would be reported as follows.

CAMPUS-ID-OF-DISCIPLINARY-ASSIGNMENT 999999003 indicates that Austin and Lea were removed to campus 003.

DISCIPLINARY-ACTION-NUMBER 001 is an arbitrary number to distinguish a particular student’s disciplinary action from other disciplinary actions of the same student. Because there are two disciplinary removals, both Austin and Lea will have DISCIPLINARY-ACTION-NUMBERs 001 and 002.

DISCIPLINARY-ACTION-CODE 06 indicates that Austin and Lea received in-school suspension assignments.

DISCIPLINARY-INCIDENT-NUMBER 001296 is the number that was assigned to the incident by campus 001. Since Austin and Lea were involved in the same incident, all four (4) of their 425 Student - Disciplinary Action records at district 999999 show the same incident number.

DISCIPLINARY-INCIDENT-NUMBER 000350 is the number that was assigned to the incident by ABC ISD campus 002.

REPORTING-PERIOD-INDICATOR-CODE 1 indicates that the disciplinary assignment was made during the first reporting period.
